Option 3: Create a custom consent form
Using this consent option you can build a custom consent form that can consist o 1 information sheet and up to 6 separate items.
The information sheet and each consent item can contain:
- An image (e.g. screenshot of a consent form paragraph)
- A paragraph of text
- Both, an image and a paragraph of text
The information screen:
- Does not require any response from the participant
- Will be omitted if no the text box is empty and no image is uploaded.
For each consent item you can choose if:
- They are shown on the same screen as the previous item
- They are shown on a separate screen, once the participant responded to the previous item
Each item will present the participant with a choice between:
- I consent
- I do not consent
